Standard Background Investigation Forms
A properly completed questionnaire is key to a complete and thorough background investigation. It helps investigators complete fieldwork in a timely manner and reduces the lengths of interviews.
There are three standard forms (plus a supplemental) used to conduct background investigations if you’re under consideration to work for, or on behalf, of the U.S. government:
